A Healthy North Coast Pilot Program

Community Bowel Cancer Nurse Navigator

Providing continuous, community-based supportive care across the bowel cancer continuum

The Community Bowel Cancer Nurse Navigator provides personalised, nurse-led supportive care for people diagnosed with bowel cancer across the Healthy North Coast region.

Working alongside GPs, specialists and hospital teams, the program complements existing medical care by providing education, supportive care, care coordination and navigation throughout the cancer journey.

Unlike hospital-based services, which are often linked to specific episodes of care, this community-based program provides continuity before, between and after appointments—supporting patients where they spend most of their cancer journey: in the community.
