Meet Sarah – Your Whole Health Nurse

With compassion, clarity, and clinical experience, I help you feel more in control of your health.

Hi, I’m Sarah, a Registered Nurse with over 14 years of experience across acute hospital care and cancer care.

I created Wellness Wayfinder to offer the kind of guidance I saw so many people needing but rarely receiving. After 10 years in a fast-paced Medical Assessment Unit, I moved into Cancer Care and that’s where I found my true passion.

Over the last four years, I’ve supported people through some of the most challenging times of their lives. I regularly see the same patterns: patients and families leaving appointments or hospitals feeling confused, overwhelmed and alone. No one should go through a cancer journey alone or unsupported.

There’s a lot of information. A lot of decisions. A lot of fear. And not always enough time or clarity to make sense of it all. That’s where this service comes in, to slow things down, put you back in the driver’s seat and help you live well with your condition.

With years of experience both in hospitals and alongside specialist teams, I speak the language of the health system and I’m here to translate it into something that makes sense for you.

Let’s Break It Down

I don’t replace your medical team, I work alongside them. My goal is to support you with hyper-personalised care in the community tailored to your life.

Definition: A Registered Nurse (RN) is a licensed healthcare professional trained to assess health needs, support individuals through illness or diagnosis, and provide education that empowers people to make informed health decisions. While RNs traditionally deliver hands-on clinical care, they also play a vital role in health education, care coordination and patient advocacy across a range of settings including hospitals, public health organizations and primary health care.

Within this nurse-led service, the RN acts as a nurse navigator, helping individuals and families navigate the healthcare system, understand medical diagnoses and manage complex journeys such as cancer care. The focus is not on providing physical or clinical treatment, but rather on offering personalized guidance, emotional support and health literacy tools to improve outcomes and reduce confusion during challenging times.

What Is Nurse Advocacy?

Nurse advocacy means you don’t have to navigate your health journey alone. I help you understand your diagnosis, treatment options, and the healthcare system making complex information clearer and decisions easier.

Who Can It Help?

Whether you’re dealing with a new diagnosis, chronic condition, or something complex like cancer, nurse advocacy supports you and your family. It’s especially helpful if you’re feeling overwhelmed or unsure how to access the care you need.

Why Does It Matter?

Healthcare can be confusing and hard to navigate. Nurse advocacy bridges the gap between medical professionals and everyday life, giving you knowledge, confidence and support. This leads to better communication, stronger care partnerships and improved outcomes.

What Is a Nurse Navigator?

A Nurse Navigator is a highly experienced Registered Nurse who works closely with patients, families and healthcare teams to support your care journey. They simplify complex health information, help you make informed decisions and connect you with resources in your community to complement your existing care.

Who Can They Help?

If you’re facing a serious diagnosis like cancer, juggling multiple treatments, or feeling lost in the healthcare system, a Nurse Navigator can be a steady guide. They support patients, families and carers to stay informed, organized and empowered.

Why Does It Matter?

Healthcare can be overwhelming. A Nurse Navigator helps reduce confusion, delays and stress, ensuring you get the right care at the right time. Their support can lead to a smoother experience and better health outcomes.
